Pathways Home

Help to keep your home

Adur & Worthing, Crawley and Horsham

Pathways Home helps you stay in your home.

We provide short-term, practical support to resolve problems before they become a crisis. Whether it’s debt, disputes, or paperwork — our goal is simple: to help you keep your tenancy.

What we offer

  • Preventing homelessness – help to stop you losing your home
  • Resettlement – support for new tenants to get settled
  • Befriending – in some areas, we also offer befriending and mentoring

In other parts of West Sussex, similar services are provided by partner organisations.

How to access

Referrals come from local council housing teams in Adur & Worthing, Crawley, and Horsham.

If you’re worried about losing your home, contact your local council’s housing team as soon as possible — the earlier you ask for help, the more we can do.

Discharge to Assess

From hospital to home

Brighton and Hove

We help you move safely from hospital back into the community.

If you’re ready to leave a mental health ward but your housing isn’t sorted, we can help. We provide a 6-week support package — and temporary accommodation if you need it — so you don’t stay on the ward longer than necessary.

Who it's for

  • Adults (18+) in Brighton & Hove
  • People on a mental health ward who are ready to move on

What we offer

  • 6-week support – one-to-one help to get back on your feet
  • Temporary housing – a furnished place to stay if you need it
  • Housing specialist – based at Mill View Hospital to help with accommodation

In other parts of West Sussex, similar services are provided by partner organisations.

How to access

Referrals must come from NHS mental health services. We don’t accept self-referrals for this service.

If you’re a patient ready to leave hospital, speak to your ward team about a referral to Discharge to Assess.

We help you move from temporary housing into a permanent home — and support you to stay there.

We also support people who need extra help to keep their tenancy.

Assertive Outreach

Help for people sleeping rough

East Sussex

We believe no one should have to sleep on the streets.

Our outreach team works across East Sussex — Eastbourne, Lewes, Wealden, Hastings, and Rother. We go out early in the morning and late at night to find people sleeping rough and offer a way into housing.

What we do

  • Find people – we go out to find people sleeping rough and understand what they need
  • Emergency housing – we help people into emergency accommodation as a first step
  • Long-term solutions – we support people to move into a settled home
