This service is open access – you can get in touch directly without going through a GP or mental health team first.
Community Connectors
East Sussex • Part of UOK / Neighbourhood Mental Health Teams
Practical support to help you connect with your community – whether that’s finding local groups, sorting out benefits, or working out what support is available to you. Community Connectors work alongside NHS Neighbourhood Mental Health Teams to help you find the right help at the right time.
Referral needed
With a referral from a professional
These services need a referral from your GP, mental health team, or another health professional. If you think one of these might be right for you, speak to your GP or care coordinator.
Not sure how to get a referral? Your GP is usually the best starting point. Tell them what you’re struggling with and ask whether any of these services might help.
